BAS-PAS Certification Permission to Post Form

Participants who attend PAS/BAS Reliability Training and are reliable with the national anchors are eligible to become certified. If certification is achieved they will be listed on the McCormick Center’s Web site as a certified PAS/BAS assessor. This form should be submitted when applying for certification/recertification and allows your contact information to be posted online.
